New pulse oximeters have advanced the state of the art in oximetry, providing an absolute measurement in real time without the need for preliminary calibration or the use of a look-up table. They have enabled applications in research, diagnosis, and monitoring of patients with respiratory and cardiovascular diseases, such as emphysema, chronic obstructive pulmonary disease (COPD) and congestive heart failure (CHF). These are conditions that affect blood flow, as well as oxygenation of arterial blood, cells, tissue and organs of the body. Finger oximeter and handheld pulse oximeters have been used in clinical settings for more than twenty years to non-invasively measure the amount of oxygen in a patient’s blood.
Medkioskinc explained to us how pulse oximeters work, which we found to be amazing. Apparently, pulse oximeters have probes that produce light at two wavelengths, namely 650nm and 805nm when switched on. This is based upon the measurement of the ratio of light absorption of red and infrared light as transmitted through the thin part of a patient’s anatomy (e.g. fingertip). Note that “nm” stands for nanometers. A nanometer is equivalent to one billionth of a meter. This light is absorbed by the hemoglobin in the blood in different amounts that depend on whether the blood is saturated or de-saturated with oxygen. The microprocessor in the device calculates the absorption at each of the wavelengths and determines the degree to which the hemoglobin is oxygenated. Since blood flow is pulsatile, oximeters can also measure pulse rate and produce graphs of the quality of blood flow. The oximeter is dependant on pulsatile flow and produces a graph showing quality of flow. The microprocessor can distinguish between static signals due to tissue or venous signals from pulsatile flow. Lung and heart disease patients often want to know the quality of their blood flow as well as oxygen saturation levels and can monitor both with a pulse oximeter. If the flow is sluggish it may be an indication of vasoconstriction or hypovolaemia, and this case the pulse oximeter may not be able to function.
We asked Medkioskinc how pulse oximeters are calibrated and were told that they are calibrated during manufacturing and on power-on that they automatically check their internal circuits. Most of the devices are accurate within 70 to 100% range of oxygen saturation with an error of +/-2%. As the saturation falls below 70% the pitch of the audible pulse signal also falls.

They do not depend on intersubject variability. Hence, it is more accurate, especially at the lowest and highest saturation levels. In experts’ opinion, accuracy of new technology has helped doctors, clinics, and hospitals in replacing traditional invasive sampling procedures, viz. arterial puncture or an indwelling arterial catheter, for obtaining absolute oxygen measurement.
